This is the top clinical position in hospice, directly responsible for all nursing and counseling services

We are looking for a highly motivated individual with top clinical skills, a passion for hospice care, the ability to manage employees in a high tempo environment. The right person for this position will be a team player, enthusiastic about working with both administrative as well as clinical staff.

Requirements (minimum):

BSN with three to five years’ experience in hospice care — three years hospice case manager experience. Management experience at the agency level preferred. Applicant must have a current WA RN license, CPR certification, reliable transportation, current car insurance, and a satisfactory driving record.

Current MDS 3.0 experience
At least 2 years of experience in the MDS Coordinator role

Manages overall care of assigned hospice patients by coordinating and providing nursing assessment, planning and care to maximize the comfort and health of patients and families in accordance with the interdisciplinary plan of care. * Must be a registered nurse with a current state license. * Must have current certification for CPR. * Must have a minimum of 1-year general nursing experience with current Hospice or Home Health . * Experience with providing pain management and IV skills preferred. * Must demonstrate a commitment to the hospice philosophy of care. * Previous Hospice/Home Health experience a plus. * Must be a licensed driver with a reliable automobile that is insured in accordance with state and/or organization requirements. * Must demonstrate excellent observation, nursing judgment and communication skills. * Must possess the ability to provide good written documentation in a timely manner. * Must be self-directed with the ability to work with minimal supervision. * Must be capable of maintaining regular attendance. * Must meet all local health regulations; pass post-offer drug test and past post-employment physical exam if required. * Must be able to relate professionally and positively and work cooperatively with other employees at all levels.
